Accepted Insurance Providers

I currently only accept Kaiser Permanente insurance.

Here's how to get started:

Step 1: Book a consultation with Erin

Step 2: You will receive 2 forms prior to the consultation. On the Pre-Consultation Questionnaire, there is a space to list your MRN with Kaiser. I will use your MRN to get the details of your benefit and provide you with that information during the consultation. 

Step 3: After each session I will submit claims to Kaiser on your behalf. You will be billed through my system for any co-pays or deductible costs. 

Out-of-Network Billing

If your insurance provider isn’t listed, the steps below can navigate out-of-network billing. On some insurance plans, you can be reimbursed by your insurance provider for the cost you pay to your therapist. Out-of-network billing must be managed directly between you and your insurance provider.

Step 1: Check with your insurance provider and ask if your plan covers out-of-network providers. If so, the cost will typically be reimbursed to you at a certain percentage. Ask if your plan includes out-of network-providers, what percentage will be reimbursed, and how to submit a claim. 

Step 2: After a session I will provide you with a superbill, a detailed receipt required to submit a claim for reimbursement. 

Step 3: Submit a claim to your insurance with the superbill provided , based on your insurance providers instructions for how to do so.


Your Right to a Good Faith Estimate

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost 

Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical  items and services.  

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. 

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service. 

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. 

  • Make sure to save a copy or picture of your Good Faith Estimate.
